Post-acute care in skilled nursing facilities is common, costly, and outcomes vary. After providing an overview of patient outcomes and existing efforts to improve the quality of care in skilled nursing facilities, Dr. Ryskina will present emerging evidence of the role of physicians and advanced practitioners in post-acute care outcomes of patients discharged to skilled nursing facilities after a hospitalization. She will then discuss implications of these findings and potential next steps for policy intervention design and evaluation.
