Quality healthcare depends on reducing the risk of unnecessary harm to patients and avoiding errors - this requires examining safety at all levels of care, from the actions of individual providers to the overarching structure of entire healthcare systems. By examining the best ways to deliver care designed to avoid preventable harms, IHPI members’ work contributes to building a culture of safety that is integral to the delivery of quality healthcare.
